5-630 stamp indicates it is a Series 5, manufactured the 30th week of 1976. One of the plastic bearing holders is broken, but that does not seem to affect operational ability unit. We were unable to test but opened the unit inspect and take pictures. Includes the mounting hardware shown - some are stainless steel, not.
